Hospitals across the US have been applying
performance improvement methodologies to prevent
patient care errors, but few have provided systematic
controls that enable sustainable results. This session
will demonstrate research which differentiates the
standard Healthcare Failure Mode and Effects Analysis
(HFMEA) Model with the Error Proofing Model.
Successful application of the Error Proofing Model for a
variety of health care failure modes that cross the
patient care continuum from pre-admission to postdischarge
will be reviewed.
After this presentation you will be able to:
- Discuss the documented evidence and importance of linking the HFMEA Model with the Error Proofing Model.
- Describe error proofing solutions that have been used successfully in the Japanese industry to “change work operations to fit human behavior.”
- Apply the Error Proofing Model to a variety of common health care failure modes that cross the patient care continuum.
