This session will present the results of 20 clinical
teams participating in Quality Allies: Improving Care by
Engaging Patients, a three-year initiative to identify and
disseminate innovative, sustainable models for selfmanagement
support in the office practice setting (both
specialty and primary care). Practical strategies for
implementing the core components of selfmanagement
support will be presented, along with tips
for developing a business case and tools to engage
patients and families in the design and delivery of care.
After this presentation you will be able to:
- Identify the core clinical components of selfmanagement support.
- Describe the business case for selfmanagement support achieved by several ambulatory care practices working with a variety of payment models.
- Discuss the advantages of, and practical strategies for, involving patients and families in the design and delivery of care.
