      This session will describe a standardized approach to medication reconciliation, both for patients coming into and going out of the hospital, using IHI'sreliability model. The presenters offer a unique perspective as a health delivery organization working on medication reconciliation across the continuum and describe how that impacts the hospital’s reconciliation approach. Leveraging the electronic medical record for all providers (hospital-based, specialty, and office-based) to reconcile medications will also be discussed.
