Learn successful methods to implement and maintain the gains of medication reconciliation within the hospital setting, and how to engage other facilities and community partners to meet the 2005 National Patient Safety Goals. Faculty will discuss the Joint Commission on Accreditation of Healthcare Organizations (JCAHO) and IHI’s 100,000 Lives Campaign goals for medication reconciliation. Lessons learned about how to minimize rework and reduce complexity for all stakeholders, how to be successful without high technology solutions, how to market the process, and how to build on the work of others will be discussed. Examples of template forms and policies will be shared.
After this presentation you will be able to:
- Discuss keys to success such as including the voice of the customer, designing a good process, gaining strong leadership support, and identifying key measurements to track and maintain success.
- Describe the JCAHO definition of medication reconciliation and the need for reconciliation at various points of care, including Emergency Department, same-day and outpatient surgery, and all transfers from one level of care to another.
- Identify approaches to engage other facilities and community partners in standardizing the medication reconciliation process.
