      This session will depict improving health outcomes for the most impoverished region of Rochester, New York, through office practice transformation. Community engagement in the work as the project addresses organizational alignment and obstacles to the implementation of Advanced Access, highly functional teams, planned care, and integrated behavioral health will also be explained.
