It is not usually difficult to understand what constitutes excellent care for patients with chronic illness. The real challenge is managing transformational change at the microsystem level to achieve highly reliable and systematic care. Building the foundation for change requires attention to team development, change management tools and strategic planning. This session will present a curriculum for change management at the clinical office level with practical tools and techniques. The goal is to achieve the benefits of the new model of care with resulting better patient outcomes.
