      This session will describe a clinic system redesign from project focused improvement efforts to standardized processes for delivering evidence-based care. The Care Model Process emphasizes the role of each member of the care team to provide reliable experiences andcare for patients. It has been implemented across sixteen clinics within a care system. This session will review the approach, tool, and case examples that can be applied in clinical practice to improve reliability and clinical outcomes.

  • Involve and engage the care team, including physicians, in designing, implementing, and sustaining reliable processes.
  • Apply the concepts of reliability to embed evidence-based care into clinical practice.
  • Leverage technology to support clinical reliability.
