This session will describe the improvement in care for patients with long-term conditions in primary care, based on the work of the National Primary Care Development Team (NDPT) across England, and how this work was spread across the whole of England, Scotland, and other countries.

- Describe the principles behind improvements in long-term chronic conditions in primary care.
- Discuss the importance of self-care and how it fits into the work of NPDT.
- Explain the methodology for spread adopted by NPDT to enable large system spread.
