This session will describe how interdisciplinary teams and focused process improvement can improve the experience and quality of care for geriatric patients, but the structure and process of such teams can take many different forms. Starting with an overview of techniques to understand and evaluate team-based, patient-centered care, three groups funded by the John A. Hartford Foundation to create and evaluate geriatric interdisciplinary teams will present their practice models.
After this presentation you will be able to:
- Identify new strategies for treating the older adult with multiple diseases.
- Identify new strategies for team building using internal and community resources.
- Utilize new tools to improve quality in transition of care, patient follow-up and functional teams.
