      This presentation will increase the learners awareness of coordinating a front-line multidisciplinary care process to create a Rapid Assessment Team that will evaluate and reduce feelings of vulnerability amongst patients, families and caregivers. After the presentation, the participant will be able to:

• Describe the process for creating a multidisciplinary Rapid Assessment Team in the acute care setting.
• Define the meaning of quality care delivery and coordination of care to patients and families in vulnerable situations.
• Summarize the positive approaches and outcomes to the patient experience with proactive care delivery and staff-to-staff mentoring and teamwork.
