This session provides an overview of the BlueCross BlueShield (BCBS) pilot demonstrations of the patientcentered
medical home (PCMH), including selected examples from local pilots. Approaches to innovative
reimbursement models and outcomes will be shared.
After this presentation you will be able to:
- Describe BCBS collaboration with physician practices and other key stakeholders in the PCMH
model.
- Identify approaches to innovative reimbursement models.
- Discuss outcomes of interest in determining the value of the patient‐centered medical home
model.
