This session provides an approachable and practical understanding of planned care and the transformative opportunities available to the small office practice. Learn how small practices can leverage their understanding of planned care to drive clinical quality improvement, decrease office waste and inefficiency, and create and nurture care teams and office vitality. The effective use of disease registries and a “blueprint” for planned care implementation in the small office practice will be discussed.
      After this presentation you will be able to:

1. Describe how to apply a planned care approach in the small office practice to improve clinical quality, decrease waste, and promote office vitality.
2. Break down complex care processes into improve‐able, scale‐able constituent workflows.
