Integrating behavioral health providers into primary care practice can bring a more comprehensive
approach to improving health outcomes. But how does a fully integrated practice function? What kinds
of issues do the behavioral health providers address? What kinds of evidence based practices are used to
promote behavior change? What does this mean for medical provider practice? This session describes
the experience of integrated care in a team based medical home model being spread through
CareOregon’s contracted network primary care practices as part of a Southcentral Foundation inspired
Primary Care Renewal Initiative.
After this presentation you will be able to:
- Design a teamābased primary care practice built around providing integrated medical and behavioral
care.
- Facilitate the development of a fully shared practice between behavioral health and medical
providers, with each gaining skills and knowledge from the other.
- Identify key behavioral health assessments and interventions based on a general understanding of
behavioral health theory and empirical research.
