Academic clinical settings that can both model and teach high‐quality chronic illness care may well be
critical to the future of primary care and the well‐being of millions of Americans with chronic illness.
Employing the Chronic Care Model, resident practices in over 50 academic settings have been redesigned
for this purpose in the Academic Chronic Care Collaboratives. Case studies will be presented as examples
of successful strategies for change.

- Describe the role of residents in effective teams that provide accessible, evidence‐based care for
patients with chronic illness.
- Explore strategies for effective change in academic settings.
