To Err Is Human, the 1999 Institute of Medicine report, highlights the need for action to reduce the
incidence of error in medicine. While our knowledge of medical error in ambulatory care remains limited,
the available evidence suggests that patient safety in ambulatory care is no better than the services
provided in hospitals. In addition to a discussion of the specific patient safety risks experienced by
ambulatory care practices, this session describes an analysis of data provided by medical groups that
utilized the Physician Practice Patients Safety Assessment.

- Explain how a physician practice or freestanding ambulatory care center can perform a selfassessment
of a medical group’s patient safety practices.
- Identify key findings from physician practices that participated in a patient safety assessment.
