Managing the care of patients with diabetes is difficult in ambulatory clinic settings. This session describes
the actions taken by a large multispecialty medical group to improve a five component bundled measure
of diabetes care from 11.5% to 28.5% in 18 months, using the Chronic Care Model as a starting point for
system redesign. The leadership and execution skills necessary to improve care across a system of clinics
will be discussed.
