How can patients and family members become better partners with the care team to improve selfmanagement
of their chronic conditions? This session discusses the New Health Partnerships
initiative, a three-year collaborative project to test effective, sustainable models for selfmanagement
support in the ambulatory setting.

- Describe the core competencies of self-management support (SMS).
- Optimize the roles in your care team to support SMS.
- Describe three delivery system models that support SMS.
- Develop a plan to spread SMS support from individuals to the whole practice.
