A grassroots network of volunteer practices has been testing the components of the patient centered medical home in a national demonstration project since January 2006. Project leaders will
describe the key components, results, and how participants can bring the concepts back to their
own settings.
After this presentation you will be able to:
- Discuss the concepts underlying the patient-centered medical home.
- Describe the results of testing the patient-centered medical home in real practices.
- Participate in the next cohort as a way to put the concepts to work in your practice with the
ongoing support of the IHI and Ideal Micro Practices faculty.
