C1: New Perspectives on Partnering with Patients and Families

Patients and families share their perspectives, insights, and experience for partnering with professionals to redesign and improve ambulatory care. Strategies for facilitating patient and family panels will be modeled, and tools to support successful implementation of patient- and familycentered care in office-based practices will be shared.

  1. Develop an appreciation for the importance of patient and family perspectives in shaping the redesign of ambulatory care.
  2. Identify various advisory and leadership roles that patients and families can assume in ambulatory care improvement efforts.
  3. Describe best practices for facilitating a dialogue with patient and family advisors in an ambulatory practice or organization.
