Studies find that having a medical home that is accessible and helps coordinate care makes a difference for patients. This session describes cross‐national initiatives, including efforts in the US and internationally, to transform primary care practices into "medical homes" that are part of a broad "systems" approach to providing patient‐centered, accessible, coordinated, effective, and efficient care.
