The work of Jönköping County in Sweden has focused on the benefits of clinical microsystems and the
importance of patient involvement in all improvement work. The NHS Institute for Innovation and
Improvement in England has focused on the value of identifying and prioritizing new opportunities to
realize and maximize the benefits of moving care closer to home. This session describes how lessons from
these countries can be relevant to you and your health communities.
After this presentation you will be able to:
- Describe how to involve the patient in improvement work, including developing a deeper
understanding of the patient context and how patients should rule your improvement work.
- Find and prioritize new opportunities to deliver care, develop a robust program of improvement
projects, and maximize the possibilities and value in bringing care closer to home.
