Collaborative self‐management support (CSMS) enables patients, families, and all members of an
interdisciplinary care team to better manage chronic conditions. CSMS changes the experience of care for
all stakeholders. It also requires a change in practice for providers and changes in the care system design.
This interactive session will begin or advance your practice's development of robust CSMS.
After this presentation you will be able to:
- Describe the core concepts of collaborative self‐management support.
- Identify the strategies for implementation and improvement of CSMS in office‐based practice
settings.
- Discuss practical ways to partner with patient and families in this improvement work.
- Apply lessons learned from the New Health Partnerships initiative to your office‐based practice.
