This session will explore how the use of triggers (or clues) to detect adverse events has proven a useful measurement over time in many hospitals. A variety of Trigger Tools exist, including the IHI Global Trigger Tool. The background and methodology for the Trigger Tool approach will be described, including the measurement to be tracked and specific lists of triggers.
