This session will discuss IHI’s recent work on the “Triple Aim”: achieving the optimal balance of good health, positive patient experience of care, and low per capita cost for a population. A frame-work will be shared along with practical insights from around the world on how to improve health and health care for a population. The Vermont Blueprint for Health and how this work is achieving the “Triple Aim” for that state will be described.

After this presentation you will be able to:

  • Describe a model and key drivers to improve the health of a population.
  • Understand how the work with the Vermont Blueprint is achieving the Triple Aim.
