C11 Linking Public Health and Health Care: Partnering to Improve Chronic Care

State and local health departments are becoming involved in efforts to improve chronic care in communities, but there is much more that needs to be learned about what their roles could and should be. This session presents some examples and engages participants in conversation about what roles local and state health departments can play in improving care.

After this presentation you will be able to:

  • Describe the opportunities for state and local public health departments and the health care delivery system to partner on improving chronic care prevention and management through joint regional efforts.
  • Articulate examples of partnerships between state and local health departments and the health care system that demonstrate improved outcomes in a community.
