The Indian Health System is engaged in an ambitious initiative, fundamentally reordering the delivery of care, both in prevention and treatment of multiple chronic conditions. This session will present an overview of the most promising ideas sequenced, tested, and implemented in participating organizations. Office practices that want to deliver more reliable and effective care will become engaged in identifying and overcoming challenges and will walk away with practical examples to get started in their own organizations.
