This session provides a step-by-step plan for implementing a prevention program for eliminating facility-acquired pressure ulcers, and a process for spread-ing this program across a multihospital system. Recommendations for rolling out the SKIN bundle will be discussed. Lessons learned by the alpha site that developed the bundle will be described, as well as the experiences of CNO leadership to spread this work across 65 hospitals with a standardized plan of care for pressure ulcer prevention.

- Utilize the four tactics included in the SKIN bundle to prevent pressure ulcers.
- Recognize the key barriers to a successful prevention program and identify appropriatere commended activities to address these barriers.
