    This session examines how one large Midwest employer, having already successfully implemented a primary care prevention and wellness model via its on-site QuadMed clinics, extended the model to include chronic care management. The “Value-based Benefit Design” process that Quad/Graphics engaged in, the infrastructure (people, process, and IT) created to support the model, and preliminary metrics of the model’s impact will be described.
