    This session will provide a review of the TransforMED National Demonstration Project, which was designed to implement and study a clearly defined model of care with a focus on the medical home and patient-centered care concepts.

• Describe aspects and goals of the TransforMED model of care.
• Discuss early learning and findings of the National Demonstration Project from the perspective of a physician participating in the project at the practice level.
• Identify the challenges facing practices that attempt major transformation of their practices.
