This session will discuss strategies to increase the effectiveness and efficiency of chronic care improvement at the practice level, as well as strategies for reaching most practice settings, especially smaller ones, and patients in a region. The early experiences of regional stakeholder coalitions focused on care quality that are implementing many of these strategies will be shared.
After this presentation you will be able to:
- Implement the Chronic Care Model and the Model for Improvement more efficiently and in a more sustainable way to improve chronic illness care.
- Describe how providers, purchasers, health plans, and consumers can collaborate in support of regional chronic care improvement.
