This session will discuss the continuing evolution of a primary care system that has resulted in dramatic decreases in overall health care costs, ER visits, and hospital days while also improving health outcomes and patient and staff satisfaction. A redesign of the entire health care system that optimizes relationships and uses asset-based approaches, and then drives a redesign of all aspects of service delivery, will also be described.
