This session introduces an emerging model for collaboration across plans, purchasers, providers, and patients to create widespread improvement across a state or region. The role of changing financial incentives, data aggregation, and building improvement support for practices will also be described. Collaborative improvement in three leading states will be discussed, along with prerequisites for successful implementation.
