This session will describe the Assess, Diagnose, and Treat clinical microsystem methodology used to engage front-line care teams, patients and families in office practice improvement and to achieve balanced improvement results. Real office data analysis will be used for a highly interactive session along with the case study of a clinical office practice. The session will also discuss how to organize a learning community of office practices to share improvement activities and lessons learned.

- Describe the tools and concepts of clinical microsystems in office practices, and the key elements of Assess, Diagnose, and Treat.
- Develop an action plan to improve your office practice using the clinical microsystem concepts.
- List four helpful resources to support improvement specific to microsystems.
- Adapt key concepts from planning a regional learning community to facilitate learning and sharing between multiple practices.
